Name the error precisely
Compare the physical card, slab label and online certification record. Write down the field that differs: year, manufacturer, set, player, card number, parallel, autograph notation or another recognized designation. A short, specific mismatch is easier to investigate than a broad statement that the entire label is wrong.
First eliminate a difference in naming conventions. A release season may be expressed differently from the copyright year, and a parallel’s marketplace nickname may not be the name used in an official checklist. If the distinction is uncertain, say so and include both possible identifications.
Separate correction, review and reholder requests
Choose the route for the problem you actually have. Replacing worn plastic does not necessarily correct an identification dispute, and a label correction does not establish that an 8 should be a 9. If more than one issue exists, list them separately and ask support how they should be handled.

Build a compact evidence packet
Take readable front and back photographs of the entire slab, plus a closer image of the label. Include the card number and any visible serial stamp. Save the certification page and your original order or purchase information. Avoid editing the pictures in ways that change color, erase marks or hide the field in dispute.
Prefer the manufacturer’s checklist or another clearly identified primary reference. Link to the specific entry and explain how it distinguishes the version. If a reverse code or design feature is relevant, describe it as one piece of identification evidence, not an infallible rule for every year.
Use a clear request format
This is a fictional request, not a submitted claim: “The label identifies Example Player, base #123. The card shown in the attached front and back photos has a visible 07/50 stamp and the checklist identifies the matching version as Gold #123. Please review the variation field. I am requesting identification correction, not a change to the condition grade.”
Add your actual order and certification numbers privately in the official form. Do not use a real person’s number from an internet example. Keep the message focused on the evidence and the requested correction; a lengthy argument about a hoped-for selling price does not identify the card.
Report promptly and follow the issued instructions
PSA’s submission terms checked September 30, 2026 require prompt inspection and written reporting of damage, discrepancies or errors within five calendar days of receipt, including applicable electronic records. They direct label errors to the Customer Request Center. If that window has passed, still contact PSA promptly and explain the timing instead of assuming no route exists.
The FAQ says confirmed label mistakes are corrected without charge and describes company-provided return instructions. Do not mail an unsolicited mixed package or pay for a new grading order merely because it is the first button you find. Save the response and follow the requirements supplied for your case.
Verify the corrected item before relying on it
When the item returns, compare the physical card with your original photographs, then check the new label and online record. Confirm that the intended field changed and that another field was not introduced incorrectly. Retain both versions of the paperwork so you can explain the history if asked.
If the card is listed for sale, pause or correct a misleading listing while the identity is unresolved. Do not promote an acknowledged label mistake as proof that the underlying card is a rare error issue. A printing error on the card and an identification mistake on a slab are separate things.
Frequently asked questions
Will PSA fix a wrong label for free?
PSA’s FAQ says confirmed mislabeled items are corrected free of charge. Submit the request and wait for case-specific instructions; an unconfirmed allegation is not an approved return.
What photos do I need?
Provide clear front and back images of the slabbed item, a readable label and any card feature supporting the correction, such as the card number or serial stamp.
Does the correction change the grade?
Identification correction and grade review are different requests. Do not assume a corrected parallel name requires or guarantees a higher numerical grade.
Can I open the slab to show the card?
Keep the slab closed. Use external photos and obtain instructions if the needed feature cannot be seen clearly.
What if the label and online cert disagree?
Capture both on the same date and describe the exact mismatch. Ask PSA which record needs review rather than choosing the more valuable version.
Is a mislabeled slab an error card?
It is a labeling problem unless the physical card independently has a recognized production error. A mistaken slab description does not create that card variation.
